All athletes must have a current physical on file in our health office to be eligible to play sports.
- Physicals must not be older than one year prior to the 1st day of the month that the sport season starts.
Example: If the try-out date is March 4, 2019 the physical must be dated March 1, 2018 or later.
- Physicals must be on file in our health office before a student can try out for a sport. DO NOT hand your physical to the Coach on the first day of practice. Physicals can be delivered in person, faxed, mailed or emailed to the Athletic or Health Offices.
- Sport physicals may be done by your family doctor or at a walk-in clinic. If you use a walk-in clinic, please download the Health Appraisal Form below and take it with you. The clinic's physician must complete our health appraisal form as the clinic's form will not be accepted. A link to local walk-in clinics that do sport physicals appears below. Call ahead to determine their fee and if appointments are necessary for sport physicals.
- Sport physicals may also be done by the school physician free of charge. The High School will offer physical exams for students in grades 9-12 on February 14 + 15 from 8:30 AM - Noon. Please sign up in the Nurses Office. The Middle School will also offer physical exams for students sometime during the 3rd quarter. Announcements will be made during school and signs will be posted when the dates are finalized.
THERE ARE NO EXCEPTIONS TO THIS POLICY.
- Physicals must not be older than one year prior to the 1st day of the month that the sport season starts.
