Kindergarten Registration

Students must turn 5 years old before December 1, 2026 in order to begin kindergarten in September of 2026.

If your child is a current (2025/2026 school year) UPK student, to finalize your registration for kindergarten in the 2026/2027 school year-

Log into your Parent Portal to finalize these steps:  

Confirm the following forms have been submitted

  •    E- Health Information
  •    F- Health Authorizations
  •    I- Previous Enrollment and Records Release
  •    L- Permissions/Agreements

-In form E, Health Information

  • Upload a NYS Health Examination form dated within 1 year of September 2026. If your child's next physical is not scheduled until after you are updating these forms, please upload the most recent NYS Health Examination for your child
  • Click here to see an example of what the form should look like. Some variations to the form may exist.
  • Upload your child's current immunization record

-In Form D, Student Contacts

  • Verify a minimum of one Emergency Contact has been added and their phone number is current 
  • Verify additional Contacts' phone number(s), address, and email address are accurate
