Health Office Information for Grade 7

New York State mandates physicals for all students in Grade 7. Physical forms are due in the Health Office within 30 days of the start of school and are valid for one calendar year.

Physical forms can be hand-delivered, mailed, emailed or faxed to the Health Office:

All students in Grade 7 must have one dose of Meningiococcal Vaccination before the start of school. If you receive a letter indicating that your child is in need of this vaccination, please contact the school nurse with any questions or concerns.
