Immunization and Health Appraisal Requirements

PHYSICAL EXAMINATIONS

Physical Exams are required for admittance to 9th and 11th grades as well as for participation in sports.  These exam forms are available online and are to be submitted to the health office before the start of school.  

 

IMMUNIZATION REQUIREMENTS - As of September 1, 2016, all students in Grade 12 will require either:
  • 2 doses of meningococcal vaccine with the booster dose given on or after age 16
  • 1 dose if your child's first dose was given on or after age 16
